It is the major issue with autos though. jumper cables are a must in an auto!

In my experience modern vehicles with electronic ignition systems are quite hard to get going with a totally flat battery. It seems that the ignition system needs a good bit of power to trigger. When trying to push start the car you can land up pushing the car around for a bit without getting it going. In this type of situation a crank handle isn't going to help much.

If not an Auto and you cannot push start like in thick sand. Jack one rear wheel in the air select 4th gear wound a recovery strap round the wheel and let rip with the ignition switched on.
You will never find me without Stroh or a 4x4 :D

Modern engines have too much compression to use a crank handle, so its push start, or as Grips stated, use a rear wheel.

For automatics, either jump start or use a POD or similar.
