Was your also at Monument Constantia?

I cannot see it not being blue, slipping for like what 100 km between 2500 - 3000 rpm every now and again I cannot think it not being the case...

Why would it have a Low Range box and the dealer arranges 4x4 events?

Yes.
In any case, a quick Google shows that you aren't alone and the 2.8 GD6 seems to have a crap clutch.

There is possibly an internal Service Bulletin for the problem, which most Dealer Principals will not admit to as it means they must repair and claim back from Toyota SA instead of fleecing you for a new clutch.

The bulletin just seems to be for the D4D as it is dated 2011 and not for the GD6?

Update:

Or the lack thereof... Called the dealership after 11:00. Said Toyota will give them feedback before 12:00. Now almost 14:00 without any feedback.

Seems as if they are thinking loud and hard about this one...

Yes, that's why I said POSSIBLY. Considering that it could be a similar issue. A Google shows plenty of clutch shudder issues with the GD6.

FINAL UPDATE

I have received feedback from the dealership.

I has been agreed that the clutch will be replaced under the extended warranty.

I am very happy with this outcome.

Hopefully this one will have an enjoyable life of 70,000 km. Thereafter an heavy duty clutch will be a consideration.

Should it have been declined it would have definitely been on the cards.

For now it is a massive relief as it would have been a big financial knock for me.

That is indeed a very lucky and very acceptable outcome. Now drive the bloody thing a bit nicer.

More than acceptable...

Will be working on the dos and donts on a clutch.

Would appreciated some technical comments/feedback.

Rule #1: Don't let SWAMBO drive. Most women use the clutch pedal as foot-rest
Rule #2: Don't use high range on a slope when towing

All I can say is goeie mur. Never seen one that bad.

I am covered for both...
1. Swambo dont like to drive the Fortuner - it is too big and it is a manual (you see, it does have an advantage).
2. I have not towed on technical terrain or overlanding with it. At most a gravel road here and there.

Will try and get some input on off-road driving practices with regards to the clutch. Sometimes there is no other way than to use it.

I should maybe point out that for the last 100 km of its life it was slipping every now and again between 2500 and 3000 rpm. Suppose that could contribute to the final look?

I'm a little concerned about the sudden failure. That clutch and pressure plate sure look like you went and towed the trailer through Baboons pass.

If you only drove gravel......something else is wrong. I would say ask Toyota about the slave cylinder and the adjustment there. The slave is a standard external slave cylinder, check the adjustment on the rod and also check the pedal travel adjuster at the pedal box.
